Audits for QOF

The latest Data Quality audits version 14006
dated 28/08/2009 is available to download from
here.
It is important to note that there are
repercussions if you do not have DLM 285
installed, so please
follow the instructions below.
All practices that have DLM 285 installed:
The Data Quality audits version 14005 and
Reporting QOF audits v14006 should be present in
your clinical audit.
This process should have occurred when the DLM
was installed. You can download the latest
version of the
Data Quality audits (version 14006) and install them. The new audits will
overwrite the previous version.
Practices that are still on DLM 280 or less:
Until Vision is updated to DLM 285, the clinical
audit will contain 2008-2009 QOF audits. Once
DLM 285 installs
successfully the QOF (v14006) and Data Quality
(v14005) audit will automatically be imported
and will OVERWRITE
previous QOF and Data Quality audits. Thus, if
you installed the Data Quality audits version
14006 before DLM 285,
the audit will be overwritten once the DLM is
downloaded and installed. To rectify this:
Re-import the latest version
of the Data Quality audits into Clinical audit.
It will overwrite the older version.
The following additions and corrections have
been made to the Data Quality audits:
• HF04 Negatives been added
• MH05 Negatives have been added
• CKD05 Negatives have been added with dates for
ACEI or ARB set at 3, 4,5 and 6 months
• AF03 Negatives have been added
• PP01 Negatives added and changed to have 1,2 &
3 month either side of diagnosis instead of
3,6,9,12 &15 months
either side of the diagnosis
• Pts with Depression in last 15 months with 1st
PHQ9 within 28 days of diagnosis have been
changed to Depression
in last 12 months
• New area (28 – Pts reported with ONLY
problems) have been added. This area is the same
as last year and reports
all the various areas where only a Problem for a
diagnosis is reported and not a Medical History
or SDA entry).
• DEP03 Negative line as it would be in QOF.
• Added Pts with NO first PHQ9 completed within
28 days of diagnosis (Depression diagnosis for
all the searches
below this line will be done since 01/04/2009)
• Added Pts with NO second PHQ9 completed within
12 weeks of first PHQ9
• Added Pts with NO second PHQ9 completed less
than 5 weeks of first PHQ9
• Added Pts with NO second PHQ9 completed within
5-7 weeks of first PHQ9
• Added Pts with NO second PHQ9 completed within
7-9 weeks of first PHQ9
• Added Pts with NO second PHQ9 completed within
9-12 weeks of first PHQ9
• Added Pts with NO second PHQ9 completed in
more than 12 weeks of first PHQ9
• Medicines Management – Active repeat master
lines have been corrected
• CKD search Pts 18+ with latest EGFR >0 and
>60/90 have been amended to only look for
entries SMALLER than 60 or 90
instead of entries equal or smaller than 60/90.
• Cholesterol negatives have been changed – the
register does not contain patients with a CKD or
Hypertension diagnosis a
nymore, but only IHD, Stroke or TIA and Diabetes
diagnosis.
• All lines in Indicator specific Expiring
Exceptions have been changed to look only for a
time period of between 15 months
and 30 months before the reference date of 1
April 2010.

Some problems have been
reported to us in the DES audits and as a result
we have made some changes:
Some minor issues on the alcohol DES audit has
been corrected.
An unfortunate side effect of the DES audits
have caused the QOF search for CHD and Beta
Blockers
to count incorrect read codes. Importing and
generating version 12012 of the DES audit will
correct this problem.

Greenwich PCT
have a local enhanced service for this year
2008/9 which involves recording the
creation of a register for all those patients
with more than a 20% risk of CVD or a 15% risk
of CHD
within the next 20 years.Some work that we did
with Greenwich PCT last year identified that the
data required to calculate this risk was not
currently being collected by most practices.
INPS have written this audit which will allow
practices to set reminders on patients where
data is
missing and do the preliminary work required
before a risk can be calculated

The CMO has recently advised all practices
of a new catch up programme for
MMR vaccination:

Following a decade of relatively low vaccination
uptake, there is now a large number of children
who
are unvaccinated or partially vaccinated with
MMR. The potential exposure of a large number of
unprotected children to the measles virus means
that there is a real risk of a large measles
epidemic.
Estimates suggest that an epidemic in England
could result in 30,000 cases, or in a worst case
scenario
more than 100,000 cases of measles in children
and young people. These children and young
people will
also be susceptible to mumps and rubella. As the
tools being developed by PRIMIS+ to support the
call
and recall of these patients will not work for
Vision practices we have written an audit to
assist in this task.
