Clinical Safety Issue – Repeat Medication Records not Showing in Vision Anywhere

Cegedim Healthcare Solutions are aware of an issue affecting Vision Anywhere from version 4.7 onwards where the repeat medication section of the records of some patients is being incorrectly presented as blank in the Vision Anywhere User Interface. 

This is due to local copies of patient repeat medication records not being able to be updated with latest available clinical information.   As a result patient encounter data may be incorrectly reported as failing to write back to the principal GP system (Vision 3) at the practice where each patient’s record is held, even though the actual writeback has succeeded.

The following is an example of what a viewer might see in Vision Anywhere when experiencing the issue. The missing repeat medications section is highlighted in Yellow under Medications.

By not having access to view all up-to-date repeat medication for patients as a result of the issue, this presents a clinical safety risk that patient care may be compromised. For example, a patient may receive inappropriate care (e.g. a drug may be inappropriately prescribed to a patient).

We are currently in the process of establishing a development fix that will resolve this issue and that will be implemented via a future Vision Anywhere software release.

In the meantime, we would ask that Vision Anywhere users are mindful of the fact that repeat medication records will not be displayed for patients affected by this issue. The repeat medication section for affected patients will be incorrectly presented as blank. In these cases, the patient (or their carer) should be consulted where possible to confirm their prescribed repeat medication to reduce the risk to patient safety.
